Effective 3/1/2023. Florida State Statute section 316.066 requires a crash affidavit be completed by everyone requesting a crash report along with a copy of a valid Driver’s License or other photographic Identification. This affidavit needs to be notarized for every request. The original affidavit is needed, no copies will be accepted. The affidavit along with the Identification can be mailed in or presented in person.
